• The email address used to contact you.  (This isn't necessary because you've already filled out the form and signed in to your Twitter account.) You don't need to do this.

• Use your Twitter username to have the other person presenting your profile agree to deactivate/delete their Twitter account. An account is required if the account is logged in and cannot be accessed.

• Enter the name of the Twitter account owner if you don't already have one.  If yes, please fill in

Fill out the form and click "Submit" to verify your security via Recaptcha whenever required in the market.  Or if not, submit the form immediately.  It's up to you.
